Traditional metal braces have been the standard for decades, but clear aligners have emerged as a popular alternative. This article examines the differences between these two approaches, outlining their benefits and limitations (1-4).
MECHANISMS OF ACTION
Braces
Traditional braces consist of metal brackets affixed to teeth, connected by wires and secured with elastic bands. By tightening the wires periodically, orthodontists apply controlled force to shift teeth into desired positions. Variations of braces include ceramic, lingual, and self-ligating systems (1-5).
Clear Aligners
Clear aligners are custom-made, removable trays that fit snugly over the teeth. Treatment involves a series of aligners, each slightly different, gradually guiding teeth to their final position. Aligners are typically replaced every 1-2 weeks, depending on the treatment plan (1-3).
EFFECTIVENESS
Both braces and aligners are effective for correcting various orthodontic issues, but they may differ in their capabilities (4-8):

– Increased difficulty in maintaining oral hygiene
Clear Aligners
– Requires high compliance for successful outcomes
– Limited effectiveness in complex cases
– Potential for misplacement or damage if not handled properly
– Additional cost for replacement aligners if lost or damaged
CONSIDERATIONS FOR PATIENT SELECTION
When choosing between braces and aligners, several factors should be considered (3-8):
